What is a health coach?

Health coaches are nurses that work with primary care providers to offer patients one-on-one support to set, implement, and achieve health care improvement goals. This is a free service offered to Monroe County Hospital & Clinics patients.

Who do health coaches help?

Health coaches work with patients who suffer from chronic diseases such as diabetes, high blood pressure, or COPD.

What does a health coach do?

Health coaches work with patients to create customized health care improvement plans to help meet each individual’s specific needs. Most importantly, the purpose of the health coach is to provide ongoing support as patients navigate their journey of chronic disease management.

The Process

Sit down with patient and discuss his or her chronic disease > Find out their personal milestones they would like to achieve > Create small, obtainable goals to get the patient on track

Plans may include referrals, information and resources, and follow-up visits with both your provider and your health coach.
