Monroe County Hospital & Clinics Social Services Department offers to assistance individuals and families while identifying or accessing appropriate resources within the community and beyond to best meet the patient’s needs. This can take place during discharge planning or for individuals interested in services offered in the Albia community and surrounding areas. The Social Service Department is available to assist with applying for assistance through programs offered though Iowa Department of Human Services, such as Medicaid or Presumptive Eligibility if someone currently does not have health insurance coverage. MCHC Social Services also provides Notary Public for the State of Iowa services.

Monroe County Hospital & Clinics Case Management/Discharge Planning focuses on meeting the patient’s continuing healthcare needs such as: Long Term Care, Home Health or Hospice services, Ambulatory or Rehab services, support groups, community agency services, community Mental or Financial Health. The purpose of discharge planning is to identify a patient’s unique needs for continuing physical, emotional, mental, social, or environmental needs and to arrange services to meet those needs. Case Management also coordinates skilled care admissions from outside facilities.
