When a patient no longer requires an acute level of care but is still not ready to be discharged home, Skilled Care may be the answer.
Some needs that may qualify for skilled care would be continued IV antibiotics, physical therapy, and occupational therapy for strengthening. Extensive dressing changes and wound care may also qualify someone for Skilled Care. When patients are in larger acute care hospitals, they are often sent to a facility providing skilled care services in or near the town in which they live. Monroe County Hospital & Clinics is proud to offer skilled care to the Monroe County area and help bring patients closer to home. We have a Case Manager on staff to assist with the transfer process and to answer any questions that patients may have.
